Statement of Completion

I, with hereby affirm that I, and solely I, as of select have completed all the required pre-class Classroom Work. I have also completed this within 1 year of my class which is scheduled on select with . I also affirm that I have seen the entire power point presentation, all the video's and completed the required quizzes. I understand that any questions I may have or clarification I may need, will be handled by my UTD Instructor during the classroom review section of my UTD class. I also understand that I must complete this Statement of Completion (Fill it out), Print It and bring it to class to sign in front of my instructor. The instructor will require this Statement of Completion for his UTD Class records. I also understand that I must have completed all the required Quizzes, which my instructor will grade and then review so that I have a 100% understanding of the test.

I fully understand that the terms of this Statement of Understanding and it is contractual in nature and not a mere recital. I further state by way of my signature I have signed this Agreement of my own free act. I also declare that I am of legal age and competent to sign this Agreement.
